Being a Clinical Supervisor as a Senior Trainee

The programme of senior (ST6+)  Resident Doctors (RD’s) acting as the named clinical supervisor for an FY2 doctor was approved in 2024 after a pilot that was run in both NHS England KSS and NHS England South West.

This pilot demonstrated that for FY2s not only is there minimal risk to taking part in the programme but actually FY2s preferred the closer mentoring presence that the Registrars provided and preferred this type of clinical supervisor relationship to the traditional arrangements.

For RD’s CS there were multiple benefits both with development of generic professional skills and supervision specific ones. It was mentioned as being beneficial for future employability and also making the RD CS feel better prepared to take on clinical supervision in the future as part of their consultant practice.

If you do the whole day’s course (modules 1-5) then this will accredit you to take on full Educational Supervisor responsibilities when you start a consultant post.

Senior Resident Doctors (RD’s):

If you are ST6 or above in any secondary care specialty with an outcome 1 on your most recent ARCP you can take part. You will need to complete your Clinical Supervisor training as well. You will need approval from your own Educational Supervisor. There will also need to be an appropriate FY2 in your department and you must be in that department for the duration of their rotation. 

You must complete the standard clinical supervisor training that all CS in the South West will complete. This is available to book on Max Course (https://www.maxcourse.co.uk/heesw-pgme/guestHome.asp) regardless of if you are taking part in this programme. CS training is Modules 1-3 of Supervisor Training, you can also complete modules 4-5 which will allow you to be an educational supervisor in due course.

FY2 Doctors must have passed FY1 with an outcome 1 and be happy to take part in the programme. If there are no suitable FY2s in your department then unfortunately you will not be able to take on the CS role.

The Consultant or SAS doctor that would have been the CS for the FY2 doctor is ‘displaced’ for the period of the programme. It is recommended that they come to the start of placement induction meeting. Then we suggest you check in with them a couple of weeks into the rotation to make sure there are not teething issues. We would suggest a formal mentoring meeting with them in week 8 and week 16 of the rotation. the displaced Clinical Supervisor is available to your for advice and also to step back in if there are significant challenges that pose a risk to the professional development of the FY2.

The displaced CS, FY2’s ES, your own educational/clinical supervisor and the Foundation Programme TPD are available for support if required.

You will be added as their CS on Horus eportfolio by the PGME team at your trust. Any issues please contact them.

This will be done predominantly by the Medical education team and the Foundation TPDs. If you have any concerns or conflicts of interest please contact them. 

Registrars who took part in this programme reported professional and personal development, including specific supervision skills that positively influenced their preparedness to take on supervision in the future. 100% who took part said that they enjoyed it , found it rewarding and found that they learned something and were better prepared to take on supervision in the future.

FY2s

This is a programme which gives senior registrars the opportunity to be a clinical supervisor for an FY2 Doctor. 

FY2 Doctors who took part in the most recent programme overwhelmingly found it a rewarding and enjoyable experience and recommend it to their colleagues. FY2s valued the closer mentoring presence that the RD CS provided and preferred this dual supervision to the traditional model.

If you had an outcome 1 in your FY1 ARCP then you are eligible to take part. Most importantly you will only be matched if you are happy to take part!

The RD CSs will undergo the standard educational supervisor training that all consultants undergo in order to take on supervision responsibilities. 

The consultant or SAS doctor who would have provided your supervision is termed the displaced clinical supervisor. They will be available to both you and your RD CS for support throughout the pilot if there are significant challenges. 

It is expected that the RD CS will be able to assist in the first instance for most problems. If they are not able to resolve any issues then the displaced clinical supervisor, your and the RD CS’s Educational supervisors, and the Foundation Programme TPDs are all available to help.

Displaced Clinical Supervisors

This is a programme that gives the opportunity for senior (ST6+) RD’s to be the Clinical Supervisor for selected FY2 doctors. They will be added to the eportfolio and it is expected that they will fulfil all supervisory responsibilities.

We suggest that you attend the start of placement meeting between the FY2 and RD CS. Please check in with the RD CS a couple of weeks into the rotation to check there are no teething issues. We ask that you have a formal mentoring meeting around weeks 8 and 16 of the rotation to discuss how the supervision is going, answer any questions and also discuss the end of placement sign off.
