Learning Objectives
- Introduce Modern Service Frameworks as part of delivering the ambitions of the 10 Year Health Plan and how the SW was represented in the development of the new CVD MSF
- Raise awareness of the 12 CVD MSF priorities and describe how these will reduce premature deaths from heart disease and stroke through action, innovation and system shifts
- Describe our SW response to the CVD MSF, including building on our SW CVD journey so far driving system action through partnerships under governance of our CVD oversight board and SW Prevention Board.
- Share how to get involved in our joint SW campaigns this Autumn e.g. Know Your Numbers, to improve health outcomes for those people living in the SW
- Focus on impact of CVD MSF on Health Inequalities and Inclusion Health
